Changes to Immediate Reporting Requirements

The process and phone number for reporting a Category A occurrence is changing. From 1 July 2019, the initial verbal notification must be reported to ONRSR by calling 1800 430 888(24 hrs/ 7 days).

This new process will replace the existing arrangement whereby initial verbal advice was provided to the ATSB. Following consultation with industry, amendments to the Transport Safety Investigation Regulations 2003 to facilitate this change have been approved and are expected to be signed at a meeting of the federal executive council on 27 June 2019 - to come into effect on 1 July 2019.  Any delay is considered unlikely.  Work should commence to prepare for the change.

The changes also coincide with the introduction of prescribed incidents, which are required to be reported immediately to ONRSR from 1 July 2019. This means all reporting will be direct to ONRSR with prescribed incidents and Category A incidents immediately and Category B incidents within 72 hours.

Category A and B written reports

Continue to send written reports of Cat. A and Cat. B occurrences to ONRSR within 72 hours. These should be submitted via the ONRSR Portal.

*Note prescribed incident requirements remain unchanged in NSW (regulation 28(2)) and do not apply in Western Australia.

ONRSR will immediately forward Category A verbal notifications to the ATSB (other than deaths involving intentional acts and some occupational health and safety matters) for consideration for investigation.  The ATSB may contact reporters separately for further details if required for a potential investigation.
